The Commissioning, Contracting and Planning Directorate have identified an opportunity for working in collaboration with Adult Social Care and the Crawley and Horsham and Mid Sussex CCG’s to take forward an evaluation of Falls projects. The Programme Manager, reporting to the Programme Director for Stroke, will work with the commissioners across social care and CCG projects focussed on prevention of falls in the community and care homes. This will involve leading on a project mapping and evaluation process with CCGs and Social Care to provide: a report which evaluates the outcomes, identifies duplication of resources, and any gaps in services. The report will inform strategic commissioning plans; identify best practice and provide indications of where value for money can be improved across care settings for older people. The successful candidate will need to be a confident leader, able to work on their own initiative and to develop relationships with key lead clinicians, management colleagues and locality groups. A proven track record of influencing and inter-personal skills alongside competence in programme/project management will be essential.
