The WSCC Shared Lives Scheme is a service that provides high quality small, family and community based support for over 100 vulnerable adults in West Sussex, both on a long and short-term basis. The Scheme has achieved ‘Good’ standards of provision as evidenced by inspections carried out on the service by the Care Quality Commission, (CQC).

You will support and monitor a number of Shared Lives Carers within an allocated caseload; you will be introducing and matching Customers to Shared Lives Carers giving consideration to their knowledge skills interests and lifestyle. You will have an overall responsibility of monitoring and supporting the Shared Lives Carers and the people they support and you will be expected to meet with them on a regular basis.

You will assess potential new Shared Lives Carers and present the Shared Lives panel for approval for Scheme membership. You will coordinate and chair annual Customers reviews. You will also be involved in the development of and delivering training workshop to Shared Lives Carers countywide, you will also undertake annual Health and Safety checks.

You will hold an appropriate qualification in Health and Social Care. Demonstrable experience of supporting people with learning disabilities and people who have Dementia and a level of care management or provider service experience is desirable. We can offer a supportive environment with regular supervision and a commitment to personal development. The role has a countywide remit, which will involve travelling to diverse parts of the county, lone working managing competing priorities of a full case load and ‘hot desking’ at various WSCC hubs, having access to a car is essential.
