Assistant Care Manager(s) ACM (s) – Working Age Service

Due to a re organisation of our service three exciting new posts of Assistant Care Manager(s) ACM (s) have been created as part of the Working Age PAT service. These posts would be based in the Prevention & Assessment Team offices in the South, West & North of our area and, although it is a county-wide post and service, successful applicants would be focused on their local area. The ability to travel independently around these areas is essential. These are full-time posts, with a requirement for flexibility about duties, although part-time would be equally considered. Applications are requested from suitably experienced workers and in particular from people who have experience of working with customers on the Autistic Spectrum. Please see attached Job Profile.

This is an exciting opportunity to work in a genuinely preventative social care role, supporting vulnerable adults, who could otherwise slip through the gaps between services. The Working Age Adults’ Service sits within multi-disciplinary Prevention Assessment Teams and offers specialist support for adults of working age who face difficulties with everyday life, with a particular focus on people with Autism/Asperger syndrome.

The majority of people supported by the service will not receive funding for long term support. The aim of the role is to support people to achieve the outcomes that are important to them, by utilising their own strengths, their support networks and the resources in the community around them. The challenges the people you will support are many and varied including homelessness, social isolation, family breakdown, difficulties finding employment and challenging or risky behaviours.

If you have a person-centred approach, are creative and enjoy empowering people to take control of their lives, then this is the job for you. You will need to have strong assessment skills and the ability to work in partnership with the people you support, their families and carers and a wide range of professionals, particularly in health, housing and the voluntary sector. You will work alongside a small supportive, specialist team to deliver effective support plans that achieve a person’s desired outcomes in a short space of time.

The role covers a large geographical area and you will be working as part of a countywide team so the ability, on occasions, to travel independently to all areas of West Sussex is essential.
