WSCC has been working with the CCG and NHS Provider organisations in developing a Proactive Care team which integrates health and social care, covering the Northern area of the county - Horsham / Crawley, East Grinstead, Burgess Hill and Haywards Heath. Following the success of these teams we are now in the process of taking integration with health to the next level with the introduction of Communities of Practice. These will bring together wider professionals from community services, social care, mental health, pharmacy and third sector based around groups of GP Practices to focus on providing coordinated care in the community for a complex cohort to reduce avoidable hospital admissions and unwarranted A&E attendance, to focus on early intervention, living well at home and to promote self-management of long term conditions.

Within this Senior Social Work Practitioner role you will work closely with the social workers and care co-ordinators to ensure good practice within social care and to provide support to all the team members.

You will act as a lead professional responsible for delivering a statutory social work service to those people who access or need the service to include assessment, planning, implementing and review this includes being responsible for leading on a specialist area of service delivery, ensuring that key developments are communicated to colleagues and that business change is successfully implemented.

You will have demonstrable evidence of providing focused, timely, sound and accurate advice and guidance and working through very complex casework, and experience of making a significant contribution to delivering and implementing change.
