We are looking for an Assistant Care Manager to join our Learning Disability Transition Team. We are a dedicated team working alongside the Community Learning Disability Teams and forms part of the overall County Learning Disability Service in West Sussex. Are you; Committed to partnership and team working? Able to work closely with colleagues to assess need and risk, and plan for appropriate support services? An effective communicator and able to positively engage with people with learning disabilities and their carers / support networks as well as other professionals? Able to understand and apply the principles of achieving good value for money and be able to achieve this in work with individuals and families? Looking for a challenge and opportunity to develop your skills? Experienced in working in either children or adult social care settings and of working with individuals with complex needs?
