As a member of the ACP2 service you will be part of a dedicated multidisciplinary team committed to the delivery of initial telephone Care Act assessments.

The ACP2 duty service provides the access point for people in West Sussex requiring advice and information on how to increase their independence, thereby preventing, reducing and delaying the need for care and support in their home.

The access point receives concerns relating to safeguarding adults, adults with sensory needs and adults and children with physical disability requiring Occupational Therapy community services.
