The Horsham Community Team is a small, friendly, experienced Team that works with adults and older people who have varied needs and outcomes to be assessed.

The aim of the role is to support people to achieve the outcomes that are important to them, by utilising their own strengths, their support networks and the resources in the community around them, and through a personal budget if required. If you have a person-centred approach, are creative and enjoy empowering people to take control of their lives, then this is the job for you. You will need to have strong assessment skills and the ability to work in partnership with the people you support, their families and carers and a wide range of professionals, particularly in health, social care, housing and the voluntary sector. Ideally you will have significant experience of working with vulnerable people.

It is essential that ACM candidates have a good academic background, good IT skills and a reasonable understanding of the context of Local Authority social care, including prioritising need within a context of finite resources. Other essential requirements include good communication skills and an ability to work in a pressurised but friendly environment.

The post is based in Horsham which covers some rural areas so the ability to travel independently to these areas is essential.
