This role is an exciting opportunity to work in partnership with both Housing and Adult Services in order to explore using our extra care schemes in a more innovative way. We are looking at enabling people to remain in their own homes, for longer focusing on rehabilitation and re-enablement in order that people can achieve their aim which is to remain independent and at home.

The role of the post holder is to ensure Adult Services make best use of existing 9 ECH schemes within the Northern part of the County by:
• facilitating and coordinating access for eligible residents to schemes across the Northern areas of the County from Burgess Hill to Crawley and Horsham.
• managing operational relationships between Adults Services Locality Teams and Extra Care Providers (the latter embracing Landlords as well as commissioned care providers)
• Promote Extra care as a viable housing with care option for adults with eligible social care needs
• Develop robust links across West Sussex (in either Northern or Coastal area) between Adults Services and Extra Care providers and other stakeholders
• Ensure that ECH contributes to - minimising the requirement for residential and nursing home placements - avoidance of unplanned NHS admissions and delayed discharges - a reduction in the number of high cost care packages / placements and delivering value for money
• Support service and market development in the ECH sector Adopting a holistic view of ECH which recognises the needs and interests of self-funding residents as well as those eligible for WSCC funding.
